Short-Term Stay After Hospital Admission

Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

The ac is dripping on the floor, the nurses are on their phones not bothering to answer or either on a "break". physical therapy is ok but that's it.                             [review] => The nursing staff and CNA’s were kind and attentive.  Housekeeping was attentive and kind to my mother.  Mom said the food was incredible!  The PT, OT and physician were prompt, kind and knowledgeable .The only downside was social work saying one thing and doing another and also those working on discharge said five minutes all day which turned into hours and hours.  Also what was told at discharge about follow up was not the same when I called the social worker back the next day.  Overall, the staff seems to care about the patients as well as the management seems to care about the staff.

                            [review] => When my mom first came to this facility they weren't perfect but now they are awful. And the administrator is the worst. They will lie to you straight in your face about your loved one refusing to be turned every two hours or bathe. She is not refusing they don't have enough clean linen. I had to go purchase my mom her own towels and washcloths because the workers would tell her they don't have anything clean. The administrator told me they saw on camera dirty linen in the hall in the middle of the night indicating she had been turned and changed. I asked to see the footage but of course she started quoting Hippa laws. What Hippa law are you breaking if there are no residents in the hallway in the middle of the night?  Miraculously, after I said call me every time she refused (no matter the time) she started getting turned and changed.
She (Kendra) never return calls and makes up all types of excuses of why she didn't return your call. I even asked her to come see me in my mom's room when I was at the facility. I waited 3 hours and she never came. She has seen me at least 2-3 times since then and has not once approached me. I also sent her an email with no response. I have decided not to call her anymore, I will just communicate with "2 Wants to Know" the next time I have a complaint and I will have pictures to back up the filthiness of the facility and all the other issues. I clean my mom's room MYSELF every time I come there. My mom is now on hospice care in the facility and I cry when I go, I cry when I am there and I cry a river when I leave because I don't know what kind of care she is going to get when I leave her there. I am starting to feel depressed just having to leave my mom in a place where administration doesn't give a damn. Every day I am emotionally exhausted because I work and then have to go to Silas Creek and be a guard dog or send somebody for me. Most of the workers seem to be trying but they are overworked, having to take care of 15-20 patience at a time. It is sad how these patience have worked all their lives and have to turn over all their assets but $70 to get treated with disrespect, cruelty and a lack of dignity.

                            [review] => This place is a 0/5, hands down. The administrator, Kendra, is unbelievably rude to the residents—it’s honestly disgusting. When I came to visit a friend who’s staying there, I heard her shushing residents during activities, telling them they were “too loud.” That’s THEIR home, they should be able to laugh and enjoy themselves however they want.

The whole building is in terrible shape. The AC units are leaking in multiple rooms, the walls are all mismatched colors, and it’s so hot inside it feels like there’s no air conditioning at all. To make things worse, I overheard staff casually talking about a resident who had gone missing—they actually LOST someone and couldn’t find him. How does that even happen?!

I wouldn’t put my dying dog in this place. The food looks awful, the atmosphere is depressing, and it’s clear no one cares. Years ago, my wife had to stay here for rehab and back then it was run by a completely different team—it’s gone straight downhill since then.

                            [review] => I  start off by saying that if I  write a worse review and  appropriate for the public to read, I would write something so bad that it would be rejected by the Catholic Church and the  even. For my experience which was recent, I noticed that the staff here cares about as much about the residence and patience as a trash man does garbage bags as a matter of fact the residence here are treated about equally as trash bags are way to a trash truck they literally do not care and if you try to get help from Staff. You have to be made to feel guilty because of the comments they make or the way they act even if you need a cup of ice water, you basically have to dread getting in a verbal argument with someone. On top of all this if you need a question answered, you're better off to use your phone in google it no one in this building was able to answer just about everything. I asked. It was pathetic. There was a few newer staff member. At least had a reason not to have all the answers one being what I think might've been a male nurse, but I can't remember. He seem to be very caring, but also didn't seem to know everything about the facility yet that he was supposed to. However, the others they were more interested in being entertained by their phones or or by Who could use the most vulgar language out in the middle of the hallways without any concern for other peoples beliefs or feelings about it.  thing I can ascertain from this type of behavior taking place is that  responsible for making sure this behavior is not allowed either. Do not care or feel like their job is so much more important that they're not concerned with the  their  treats their residence like garbage. In my opinion, the only reason that people will act like this is if they have no one holding them accountable. Such behavior so that it's made stop. This is up to the people in charge so whoever is in charge in this  they need to get off their ass about it. There was one elderly nurse very elderly nurse that when I was inquiring about my family  condition acted as if I had just interrupted her most important episode of a soap opera and that it wasn't her job to answer family member or residence questions. She was just more concerned with the computer. She was sitting at and remain sitting out for most of the time I observed her. I'm not sure. Maybe she had an assigned seat but I know my concerns were definitely nothing. She was worried about. After being in this facility for a short period of time, I'm so mind blown that I'm not even sure what to say about it. The  are basically useless. Some of the staff members act as if they're entitled to certain types of treatment other staff members act like they're trying to do their job but do not have the help That they're needing so even the ones that do Care are not giving any support to know how to help people when they ask for help . This cycle in this building is as vicious as piranhas devouring a carcass in the water. I wouldn't recommend this place to be a slaughterhouse for cattle and if it was, if it was the same people working in the slaughterhouse, they'd have to shut it down too because they're so sorry. I hate to say such cruel things but after things I've seen and the lack of care and compassion from people who are paid to literally show  I'm so sick to my stomach every time I think about it that I'm literally losing weight at an alarming rate according to my doctor lol I know that if I get to the age, I have to be putting a place like this I'm rather someone  put me in the zoo  the Tigers are pinned up and allow me to go out that way then  thinking that people are there to take care of me knowing all they're there to do is satisfy their own needs and get a paycheck and it doesn't matter who you tell you have an issue about they may tell you to your face at that very time that they will handle it and it won't happen again, but I believe all they do is walk into their fancy officers some kind of food they already had ordered and was never intended on helping you shortly with it's true what they say that shit rolls down here so if your people in charge of the place, don't give a shit what's going on then obviously the rest of the people that worked there are not gonna give a shit whether or not they do their jobs correct so who can you really blame? It's sad that all those people that should be getting took care of to the best of those  members abilities are laying there even after they  themselves for hours just hoping that  same day will come in and change their whole bed and clothing. I know from the room where I was at. Smell urine so strong. It was like I had urinated on myself if I had been as old as some of them, I probably would have thought I'd urinated on myself. When the supper trays and lunch trays come out, you can't tell what the food is because when you mix it with the odors that are always up and down the hallway, the lunch and supper trays smell like pig slop urine and feces!

You have aides cussing the nurses out like they own them and the nurses will just stand their like her brain is trying to reboot, no disciplinary action is ever taken , when I was in town visiting I myself noticed that anytime you needed assistance all The staff hides it seems like or if you get lucky and find one their playing on their phones with earbuds in their ears not listening or paying attention if any call lights were going off. As far as upper management oh my lord I seen the same person all over the building all day actually doing something that looked important and trying to help residents if they had any questions, and then their is one that looks like she rode in with the dinosaurs and they forgot her when they left she looks that old, so I wonder what her job is holding the foundation of the building down or what because I don't remember ever seeing her move to do anything except waddle outside to smoke and the back to that same chair. As far the big dogs apparently they prefer to stay on the porch and not work with the rest of the nursing staff in helping make changes for the better, or they just don't have a clue on what their doing or even how to do it, they remind me of the movie clueless, because they would rather put everything on an employee that looks fairly new and somewhat lost but still trying their best! seems like to me their always trying to find a new employee to be their patsy so when state pops in they can say well it was their responsibility to get it done... yea I was eavesdroping and over heard the dinosaur lady tell another staff member that the person I assumed was new still hasn't been shown certain things and they had even mentioned it to upper level and upper management keeps brushing them off and ignoring them. So I myself hope and pray the new employee runs very far away from this shinking ship because they actually seem to really truly care but can't make a difference working with all these damn pirates because they apparently only worry and think of themselves not even the residents care is important to them I've seen first had so many times. In my opinion if the owners had half a damn brain they would fire everyone of them and start completely fresh or just place all the residents in actual facilities that care and have great staff and bulldoze this bitch over And build a whole new facility since this one is so tainted by lazy, disrespectful, clueless, incompetent, poor excuse for healthcare workers, non caring , heartless money hungry people that don't give one cent about taking care of someone the way the deserve to be and respected, no one in hear has the heart and compassion it takes to do this job except for the one person I observed Running them self to death to do their job and assist any resident thats call light was on or stopped them in the hall. I mean seriously how much evil can be packed into this one facility

Let’s say I made a terrible mistake in thinking that my family would be taken care of here! No wonder I have heard it called the “Creek” , because if you get through in your going to drown for sure. This place is so ridiculous I would not throw a fish in it. The staff use the worst vulgar language I have ever heard amongst healthcare professionals. I pray in my car each time I leave. The only plus is if you hear them cursing at least you know you can find some help. May still take 30 minutes to get a cup of water but at least you don’t feel like you are in an abandoned building. Staff basically seem to operate at their own free will with no direction not even from the administration. Some of the managers I have noticed try but one or two of them can’t do everything. I’m not sure there is the faintest sign of structure aside from the physical structure of the facility. I have seen and heard aides talk terribly to the nurses and nothing is ever done. I would sum it up as a free for all, with very limited knowledge to go around and my years in management tell me that no one here is taught much more than where to use the restrooms. If you have anywhere to take your loved one, even a homeless shelter, you are better off. I have heard that they are a five star facility, must be because they put up star shaped stickers on each hall. I would not have gave them one star but the site made me. They truly need some actual training, some discipline for sure, and definitely a lesson in how to act like the professionals they are insulting. Nurses are true angels but I guess when a place like this plucks their wings they are just in survival mode. The definitely deserve better, must be a really good salary range to submit yourself to this kind of…. Whatever it is!

                            [review] => I was very pleased with the overall experience at Silas Creek Rehab. My mother went there after being at the hospital and I was a little apprehensive after seeing the reviews, but I felt that the overall level of care she received was excellent.

The administrative staff was responsive and accommodating, the PT and OT staff (Mark and Alice in particular) were outstanding, and the nurses and CNAs were very caring and willing to help at all times. The one issue we had with a CNA was rectified immediately after we brought it to the staffs attention.

My sister broke her hip and because of the 5 stars, she chose Silas Creek Rehabilitation Center. We did not get a place that would merit 5 stars. She fell twice while she was there even though in our meetings only one fall was recorded. She didn't sleep many hours because of the noise during the night. Some of the nurses were caring and friendly but only 2 or 3; the rest were unsanitary, uncaring and even rude at times. She was often given conflicting orders from the staff, such as one nurse told her that she did not have doctors permission to get out of bed without a nurse but when she called the nurse, she was told that she needed to get up herself because she had been there long enough to accomplish that; later that night she got up to go to the bathroom, fell and had to be taken to the ER.
One day she was picked up and taken to a doctor's appointment. She was left at the front door. When she went in, she found out it was the wrong building. The ride came back but it was too late to go to the other building.
There was also a problem with her medication. One medication that she was supossed to get daily for her eyes was not given for several days until my sister let her doctor know. She has asked several times about the medication without results.
I also had problems getting answers to my questions when I called. Communication was not fluid and vague at times. I am glad she has come home and is doing well now.

They have activities. My uncle has to do exercise three times a week. They have an exercise room. a dining room, and a conference room. It's very, very well organized. Food seems to be good. He has no complaints. The only thing he complained of is that it's not enough.

There brochure talks about from the moment you get there you are welcomed.  Ha, that’s a joke the staff was nothing but rude and acted as if we were bothering them.  From that moment on I felt uncomfortable leaving my mom but knew she needed rehab.  By 7am on Saturday we were working on a plan to get her out, which was an ordeal.
Warning rude staff, place is not clean, staff does not use gloves when coming and helping with bathroom!  There are so many other things that happened in less than 24 hours but way too many to list.  Do not take your loved ones here!

                            [review] => Horrible place. I was there personally in the month of March, 6th til the 16th, 2024. Verbally and physically abusive CNA's. This is 2024... nobody should have to lay on a pee pee pad/blanket all night or all day.  Was told they were out of dry pads!  Some of my clothes went missing. Had to call the front desk/office to get stuff a couple times.
I guess that's normal?  Eileen (Philippino therapist lady) was excellent in every way. Very professional, thorough, consistent, motivated me. Speech therapist was ok. I did enjoy playing the therapy exercise games with other patients.  I left 2 weeks early with doctors full permission. Dr. should have put me on blood thinner. March 16th I left that horrible place. Had massive blood clots in my lungs less than 9 days after I left. Went via ambulance from home on March 31st. Blood clots were the kind that kill you according to the ER Dr. at Forsyth Medical Center on morning of the 31st of March.  Rarely saw a nurse at Silas Creek Rehabilitation, but that's normal. This place is horrible! My mother was there for two weeks and two days and I have never been in a position where no one on staff could prove and answer to how she was progressing. There were no daily updates and I had to request to speak to a doctor and that only occurred once and was over the phone when it happened.

We moved her home and in two days time have seen a world of difference with her progress.

This facility approves to being a money hungry facility who places their first priority on money and no priority on patient care. The state definitely needs to shut this place down. I feel terrible for those still there who have no one to advocate for them!!

State of NC.....please listen and investigate this place! People deserve better!

When I got there I told the lady in the office that I was there to take my dad home.  She was very rude. I proceeded to my dad’s room and the entire place looks like a prison. They only give the patients water, tea, cold coffee, and milk and if they want anything else they have to pay for it.  My dad was in the small bathroom that is in the room he shared with another guy and three nurses busted in on him without knocking. I said something about it and one of the nurses who was a blonde headed young girl with glasses got up in my face saying, “If you would’ve been at the hospital with your dad he wouldn’t be here”. That really pissed me off and I told her to back up out of my face and let me get my dad’s stuff together.  As I was getting it I noticed a patient’s belongings in the closet that wasn’t my dad’s or his roommate’s and when I mentioned something about it the girl got loud with me and two other nurses had to hold her back because she was trying to fight me.  This place really needs to be shut down.  They are unprofessional and the beds are so outdated like something was 1950 or 60s. Me and my dad was escorted out of the building. This person (CS) was looking at her computer. Clearly she saw me standing there. When I said excuse me, she loudly addressed me with an attitude and said "WHAT DO YOU NEED?". Clearly she was bothered by my disrupting. I then asked again for a nurse and she yelled again as she said "I AM BUSY AND I AM NOT A NURSE!" I then told her, my client is in need and I am unable to assist him. I asked her why her attitude was so bad and as she brushed past me, I told her I feel sorry for the other helpless souls in this facility. When we go to the patients room, she also had an attitude with him! He even said she is so nasty and her comments to him as she put her hands on her hips were "REALLY OH REALLY". I told her we didn't need her and that I would file a formal complaint. As someone that works with the state, it pains me to go into a facility as a VISITOR and get treated in such a way. I fathom to think how the patients are being treated. The only bright spot was the actual nurse that came to assist us, was very kind and was able to take care of the clients needs. I would strongly urge you to RUN away from this place if at all possible. My father-in-law was transferred to this rehab facility directly from Wake Forrest Hospital. THIS IS NOT a "5 star" rated facility. I cannot fathom how this rating was assigned. If you see that rating, it is wrong. At most it should be rated "1 star". Ultimately, the experience was horrifying, the conditions deplorable, and my father-in-law was there less than three hours before we pulled him out! This place was SO bad I can't imagine any family member putting someone there if they cared for them in the slightest. First, we were surprised when we arrived that they were replacing the floors in the hallways. That itself is probably ok, and I'm glad this update is being made to the facility, but what I saw behind where the baseboards had been removed was not good -- black mold. They were also laying chemicals and floor adhesive down right outside of patient's rooms with the doors open. There was no sealing off the area, dust was flying, and it was extremely loud at 10pm at night (yes, they were still working at 10pm). The room where he was placed was barely large enough for one, but housed two patients. It was possibly 12' x 12' or maybe, if I am being generous 13' x 13'. The tiny bathroom (smaller than our C-class RV's) is expected to be used by four patients from two rooms. His roommate complained the entire time we were there about the conditions of the place and how he couldn't wait to get out of there. The nurses needed to perform a skin assessment upon his arrival (which is normal and expected) however, instead of keeping private parts covered, they proceeded to strip him completely naked with no regard for privacy at all. He has had many skin assessments in the hospital and the nurses have always been very careful to keep private areas covered which gives the patient dignity and privacy. When looking around the room and the facility as a whole, paint was peeling off the walls, doors, and everywhere. When looking into individual rooms while walking down the hallway, it looked like something out of the 1950s that had never been updated. Old equipment, beds and furniture -- felt more like a prison than a modern care facility. The nurse going room to room with nightly medications seemed to be confused as to what should be given. She was also confused and did not understand the forms we signed when entering the facility or the form we signed when we pulled him out. For example, the nurse was telling us to sign where it clearly said physician's signature. We caught several other mistakes that, if we would not have been paying attention, would have been incorrect on the forms in other places. These are forms they use everyday. If they don't understand these, what else is not understood or sub-par in the care. We found out the next day from the hospital that the rehab facility should have given us discharge papers and information. Simply one more item where the ball was dropped. There was NO way we could leave him there or recommend that anyone is ever placed there. We had to get out and get out as soon as possible. We would figure out next steps later. This place needs to be shut down and "rehabbed" top to bottom.

Well he did get some PT, the PT staff was helpful he didn't get much of it. In fact on the weekend and on second and third shift they completely igroned him ,let him lay in the bed ,would not help him open his drinks nor give him a straw even know the doctor orders said it was fine.There was no communication between shifts, and they would not help him put in his hearing aids nor help to charge them.Dis not see or meet with any administration from that facility. They put everything on the social worker who in fact was very good but you could also see she was stressed running around trying to take care of everything.
They also put him in a diaper and he has never been in a diaper. Staff was made aware of this as well. They would not even help him eat his food, knowing his hands have bad  arthritis  . The first shift staff was much more easier to work with. Thank you to the ones that have  helped .

He had been in an acute rehab center after having a stroke who told us he would never be able to walk and would always be on thicken liquids and would have to eat food that had been processed to a soft mush.  I didn’t accept their report and had him admitted to Silas Creek Rehab.  There rehab dept was AWESOME!  He is home now, he walks with a special walker and can drink regular liquids and eats regular food!  He wasn’t always the easiest person to deal with and the nursing staff overall were very patient with him.  I am eternally grateful to these folks for the care they gave him!

They have new management, directives, standards of care, etc. and has made some major changes. While the facility is a little old, the staff is experienced and well educated. So far, my mom has been very impressed with the center, but that was just during the first day after being transferred from the hospital. I will update more during her stay there.

                            [review] => This is a great place to take someone if you want them to know you hate them and wish they were dead.

There are a few staff members who are nice and try their best to help, but the majority of the staff jut don't care. They will take an hour to answer a call light to assist a resident and get mad that the person wet the bed...

I suspect this place used to be a good place judging by reviews from two years ago, but this place is definitely not that place. They do more harm than good as a short-term/transitional rehab center.

The rest of this review is just a list of things that I remember we had to deal with in our three weeks at the facility that will hopefully deter you from ever stepping foot here:

1. Due to staff not assisting in a timely manner, my dad had to wear a diaper.

2. My dad couldn't use any of the bathrooms they had available because they are too narrow and the handicap rail is slanted.

3. There is one bathroom that could accommodate him but the staff would not let us use it because the bathroom is for employees only(despite not having any signs stating so and having a higher toilet seat and handicap rail that would work great for someone with a physical disability)

4. While smoking is allowed, they tell you not to bring a lighter, and they will confiscate your lighter if they see you have one. (have to hide cigarettes and lighter in room for anyone who smokes at the facility)

5. Cigarettes that are turned in to the staff are stored wherever they feel like. Most of the times they had my Dad's cigarettes stored in the medicine cart.

6. Zero accommodations were made for my dad to use his electric scooter. He couldn't use the wheelchairs they provided due to back pain and being unable to steer them effectively and the staff would not let him use his scooter.

7. Can't get any assistance when food trays are out. If you have to use the bathroom, nobody will assist you.

8. When bed sheets are changed, residents are not allowed to get on the bed, and staff will take hours to get new ones on there. This is a big problem if you have someone who uses a bedpan due to the terrible bathrooms. Even if you make staff aware of the issue they will not try and help.

9. They will try anything they can to squeeze as much money out of you as they can. It took two weeks for them to schedule a care plan meeting and were quick to find any way they could to cancel it and reschedule.

10. Along with #9, they will try and weasel their way out of spending any money. Sure they don't do anything like outright lie to you, but they will try to find any loophole they can.

I feel absolutely terrible for sending my Dad here for rehab. Don't make the same mistake I did. Its easy if you are not the resident to look past all the horrible things that happen here out of wishful thinking. Nursing homes in general have a bad reputation and as such its easy to pass off some things as just something that you have to deal with, but you don't have to deal with it. Make homes like this accountable for their actions. Don't let them draw insurance money for doing poor care.

My father asked for the nurse to turn it off around 11 PM so he could get some sleep and the nurse refused! My parents spoke to administration the next day and they said they would turn the TV off late at night for him but again the next night the TV was NOT turned off again. How can my father possibly recover and gain strength when he is refused a good night’s sleep in your facility, you should be ashamed of yourselves. We removed my father after the second night so he could get the rest and CARE he needed to strengthen himself for his next battle, chemo and radiation. Also there was no hand sanitizers or gloves being used with my father, which was very dangerous to him and his immune system that is very weakened.  DO NOT RECOMMEND THIS FACILITY!!!

It takes 45 mins to an hour for them to answer call lights. However the sad part is medicaid patients have no choice but to stay here. The rooms are so small. Trash cans are running over. There are some good nurses but they are over worked due to mismanagement. Just a sad place. Thankfully my brother gets to come home soon. I would not recommend this place tp anyone.

If you ask them nicely, they make some excuse like they're having to take up lunch trays or doing something else. After 3 hours, family members had to change the bed themselves.  Trash cans overflow; no hand sanitizers placed in hallways for staff and visitors to use; very cramped quarters when 2 to a room.  No place for visitors to sit or stand.  Inadequate help to get patients in and out of bed or to help them with dressing, etc.  I would not recommend to anyone.

When we requested meds to be changed, our requests were often ignored. He stopped eating and drinking, slept all the time. An IV was never provided. He was finally rushed to the hospital with low vitals. My father was severely dehydrated, had to be intubated, and almost died. Within 3 days of PROPER medical care in the ICU, he was back to normal.

                            [review] => Kindred Transitional Care and Rehabilitation at Silas Creek provided, from the initial contact while I was still in the hopsital until my discharge on December 19th, 2012,
totally professional and personal care. Each member of the team made me glad that I had selected this facility. I had personal interaction with the majority fo the team and they knew my name on sight. The recognition by them helped keep my spirits up.
As for the "Therapist" (all of them), they worked closely with me and adapted the care plan which would meet my goals and needs. From nursing, I again received complete care!
The 'quality' score of "very good" allows for the continuous striving for improvement, goals updating and total team work!

Silas Creek Rehabilitation is top notch in their patient care , and family care. My friend has been there for 1 year, and has made great friends with the staff, and other patients. It is clean also

The nursing staff and CNA’s were kind and attentive. Housekeeping was attentive and kind to my mother. Mom said the food was incredible! The PT, OT and physician were prompt, kind and knowledgeable .The only downside was social work saying one thing and doing another and also those working on discharge said five minutes all day which turned into hours and hours. Also what was told at discharge about follow up was not the same when I called the social worker back the next day. Overall, the staff seems to care about the patients as well as the management seems to care about the staff.
